What You Need to Know
Insurance plans vary widely. Please review the following before your appointment so you're fully prepared.
Know Your Coverage
Not all services are covered. You are responsible for any services your plan does not cover and will be billed accordingly.
Co-Pays Due at Visit
We are required by your insurance company to collect all co-payments and deductibles at the time of your office visit.
Bring Your Referral
If your plan requires a referral from your primary care physician, obtain it and bring it before your appointment.
Pre-Authorization
Some plans require pre-certification or approval before lab work or procedures. Confirm with your insurer beforehand.
Designated Laboratory
Some insurers require a specific lab. If you don't inform us, you may be responsible for any charges incurred.
Preferred Pharmacy
Check whether your plan requires prescriptions filled at a particular pharmacy before we call in your medication.
